Comprehensive Evaluation

What is really going on?
You may be an employer with suspicions, a parent, friend or loved one. You may be questioning yourself or wanting a second opinion.
The best way to find out if addiction is the problem is by doing a Smith and Jones 3 to 5 day Comprehensive Assessment.

What is the exact extent of my addiction? 
The assessment can also give peace of mind to family and friends who are concerned about someone. This service is designed for people who are not sure about the extent of their alcohol and/or substance abuse, food or gaming issues and/or are not sure whether they require treatment.
 
The evaluation is conducted by the multi-disciplinary treatment team and will include the following:

• Unique life skills evaluation
• Family and relationship evaluation
• Medical evaluation
• Psychiatric evaluation
• Nutritional evaluation
• Psychological assessment
• Written and Telephone Communication with Referral Source if requested
• Forensic Urine Screening
• Comprehensive blood tests
• Collaborative Family Session
• Clinical Summary and Treatment Recommendation provided upon completion
